GLP-1s May Expand into Cancer Prevention and Treatment

Background:
GLP-1 receptor agonists are no longer just a diabetes and weight loss story. Their rapid expansion is outpacing employers’ ability to maintain affordable and sustainable pharmacy benefit programs. Some large self-funded health plans report that GLP-1 therapies now account for as much as 20% of total prescription drug spending. Yet emerging research suggests that these medications may be delivering benefits that extend far beyond weight management.
GLP-1 for cancer prevention?
A recent ASCO study found that patients receiving GLP-1 therapy were 38% to 50% less likely to develop lung, breast, colorectal, or liver cancer. Among individuals already diagnosed with prostate, pancreatic, or kidney cancer, GLP-1 use was associated with slower metastatic progression.
The common thread appears to be inflammation. GLP-1 therapies help reduce inflammatory activity, regulate insulin signaling, and may disrupt biological pathways that support tumor growth and spread.
And cancer may be only the beginning.
GLP-1 therapies are already approved for type 2 diabetes, obesity, cardiovascular disease, obstructive sleep apnea, and chronic kidney disease. Additional indications under investigation include osteoarthritis, MASH/MASLD, substance use disorders, neurological conditions, and PCOS.
What does this mean for employers?
For employers, this is no longer simply a question of managing the cost of a single drug class. It may signal a broader shift in which one category of therapy influences many of the most prevalent and costly chronic conditions within a health plan. Strategies built around the assumption that “GLP-1s are primarily diabetes drugs” may no longer reflect the evolving clinical reality.
